The “Major Change Request” form is used by students who wish to change their current major.

The “Course Withdrawal” form is used by students who wish to officially withdraw from the core or major-related courses during the Add/Drop period.

The “University Withdrawal” form is used by students who intend to discontinue their studies and formally withdraw from the university.

The “Leave of Absence Request” form is used by students who need to temporarily suspend their studies due to personal, medical, or other valid reasons.

The “Student Clearance” form is used during the withdrawal or graduation process to verify that the student has cleared all academic, administrative, and financial requirements.
